Nine directory and review sites, led by Google Business Profile, Healthgrades and Zocdoc, supply most of the answers AI engines give about dentists, according to 5W's May 2026 audit of ChatGPT, Claude, Perplexity and Google AI Mode. In the same audit, the eight largest U.S. dental service organizations did not rank first for their own brand name on more than half of the engines tested.
Key takeaways
- Rock Health's survey of 8,000 U.S. adults, fielded in December 2025, found 32% had used an AI chatbot for health information, up from 16% a year earlier.
- 5W's audit found the same nine properties cited again and again: Google Business Profile, Healthgrades, Zocdoc, Yelp, Vitals, WebMD, ADA Find-a-Dentist, 1-800-DENTIST and RateMDs.
- The first edition's projected GEO cost curve, its AI-addressable market estimate and several vendor accuracy claims were removed because they could not be sourced.
- A dental practice can improve its AI visibility by completing directory profiles, publishing claim-dense provider bios and checking its robots.txt. The playbook below lists ten steps.

What changed since the first edition?
Six statements in the first edition were removed or made more precise in this update because they could not be sourced or were projections. The audit findings themselves are unchanged.
| Topic | First edition said | Update |
|---|---|---|
| Consumer AI health use | 32% of U.S. adults used an AI chatbot for health information in 2025 | Confirmed. Rock Health's 11th Consumer Adoption of Digital Health Survey (8,000 adults, December 2025) found 32%, up from 16%; ChatGPT was used by 23% of all respondents and Gemini by 15% |
| GEO cost curve | Citation cost rises 60 to 90% a year; $250,000 now versus $1.5 to $2.5 million in 2028 | Removed. These were projections without a source |
| AI-addressable market | $120 to $140 billion of the dental market flows through AI-influenced channels | Removed. The $185.4 billion dental services figure is Precedence Research's projection; the AI-addressable split was 5W's estimate and is not shown |
| Diagnostic AI accuracy | Specific accuracy gains for Pearl and VideaHealth | Removed. These clinical performance claims were not sourced and are outside this report's scope |
| Adoption by practice size | 72% of 50+ location chains versus 18% of solo practices | Removed pending a named source |
| Crawler advice | Unblock GPTBot, ClaudeBot, PerplexityBot and Google-Extended | Narrowed. Check that GPTBot, ClaudeBot and PerplexityBot are not blocked by mistake; Google-Extended controls AI model training use, not Google Search or AI Overviews |

Which directories supply most dental AI answers?
Nine directory and review properties supply most dental AI answers, according to 5W's May 2026 audit across finder, decision and specialty questions. Individual practices appear inside those directories but rarely above them.
| Property | Where it appeared in the audit |
|---|---|
| Google Business Profile | Anchored finder questions such as best dentist near me through local-pack data |
| Healthgrades | Second or third slot on finder questions; also appeared on procedure questions |
| Zocdoc | Alternated with Healthgrades on finder questions |
| Yelp | Appeared on review-heavy questions |
| Vitals | Long-tail finder questions |
| RateMDs | Long-tail finder questions |
| 1-800-DENTIST | Long-tail finder questions |
| WebMD | Clinical explainer questions such as implant cost or veneers versus Lumineers |
| ADA Find-a-Dentist | Trust source on finder questions; ADA's MouthHealthy.org covered parent and pediatric questions |

How did the largest DSOs perform in the audit?
The eight largest U.S. dental service organizations did not control their own AI answers in 5W's audit. The audit tested Heartland Dental, Pacific Dental Services, Aspen Dental, Smile Brands, Western Dental, Great Expressions, Dental Care Alliance and InterDent on four engines.
Each organization's own website ranked first for its own brand-name question on fewer than half of the engines tested. Healthgrades or Zocdoc profiles of its locations often ranked above the corporate site, and none of the eight had a Wikipedia entry deep enough to anchor an answer or a complete Wikidata record, according to the audit.
Why do dental practices lose AI answers to directories?
Dental practices lose AI answers to directories because directories publish short, structured facts and most practice websites publish marketing language. The first edition listed six reasons, summarized here.
- Consumer AI use grew faster than dental marketing changed. Rock Health found AI health use doubled in a year.
- AI Overviews answer many question-style health searches before a patient sees any practice site, so the first impression of a procedure or price comes from an answer, not a website.
- Zero-click behavior hides the patient journey. Standard web analytics do not show whether an engine named your practice.
- Dental marketing budgets are split among paid search, SEO retainers and directory listings, and none of those covers structured provider facts or entity records.
- State dental board advertising rules and ADA guidance on AI-generated marketing have not kept pace with the platforms, so practices wait.
- Practices are careful about privacy in clinical AI and sometimes extend that caution to public discovery content, which is public by design.
How do AI engines read dental information?
AI engines extract checkable claims, so a profile that states credentials, school, year, insurance and review counts is easier to quote than one that says a dentist is passionate about smiles. The first edition's example was a profile listing degree, school, graduation year, memberships, accepted insurance and review count.
- Schema.org markup for Dentist, FAQPage and Article pages helps engines parse a page as a structured entity.
- A Wikidata entity with sameAs links to the Google Business Profile, Apple Business Connect, Healthgrades, ADA Find-a-Dentist and the practice website ties the profiles together.
- A robots.txt file that blocks GPTBot, ClaudeBot or PerplexityBot by default stops those crawlers from reading the site, and some older or security-focused site configurations do this without the owner knowing.
How can a dental practice improve its AI visibility?
A dental practice improves its AI visibility by making providers, procedures, prices and insurance easy to find and quote on its own pages and on the directories engines already cite. These ten steps apply to independent practices and to DSOs.
- Fill in every field of the Google Business Profile: categories such as dentist, cosmetic dentist or pediatric dentist, services, accepted insurance, hours, same-day and emergency appointment availability, and recent photos. This profile anchors finder questions in the audit.
- Claim and complete the profiles on Healthgrades, Zocdoc, Yelp, Vitals, ADA Find-a-Dentist, 1-800-DENTIST and RateMDs with matching name, address, phone and provider details. These are the properties engines cite most.
- Write claim-dense bios for every provider: degree, school, graduation year, state license, specialties, languages, society memberships and accepted insurance. Facts can be quoted; slogans cannot.
- Publish a page for each procedure, such as implants, Invisalign, veneers, root canals and pediatric care, with the first sentence answering one question and an FAQ block below it.
- State price ranges where state rules allow, labeled as examples with what affects the price and whether financing is offered. Questions about dental implant cost are answered from pages that show ranges.
- Add schema.org Dentist, FAQPage and Article markup, serve pages as server-rendered HTML and validate with Google's Rich Results Test.
- Register a Wikidata entity for the practice and each principal dentist, then add sameAs links from the practice website to the Google Business Profile, Apple Business Connect and directory profiles.
- Check robots.txt and any security plugin for blocks on GPTBot, ClaudeBot and PerplexityBot, and remove blocks you did not intend.
- Collect reviews and reply to each one without disclosing patient information. Check your state dental board's advertising rules before using testimonials or before-and-after photos, and consult counsel if unsure.
- Publish bylined articles in dental trade outlets and local news, and test monthly. Ask four or more engines 20 prompts per ZIP code, such as "best pediatric dentist near [neighborhood]" and "dental implant cost in [city]", record which properties and practices appear, and compare month to month.

How was the audit built?
The audit is 5W's May 2026 test of finder, decision and specialty questions across ChatGPT, Claude, Perplexity and Google AI Mode, with the eight largest U.S. dental service organizations tested separately. Findings are directional estimates modeled from observed engine behavior, not query logs.
Scope and update details
The audit covered eight procedure categories: general dentistry, cosmetic dentistry, orthodontics, implants and prosthodontics, endodontics, periodontics, pediatric dentistry and oral surgery. Market sizing in the first edition came from Precedence Research and other market-research firms, and consumer AI behavior came from Rock Health. This October 2026 update did not re-run the audit. It confirms the Rock Health figure, removes projections and claims that could not be sourced, narrows the crawler advice and expands the playbook.

Frequently asked questions
Why are dental practices invisible in AI answers?
Nine directory and review properties, led by Google Business Profile, Healthgrades and Zocdoc, supply most AI answers about dentists in 5W's audit. Practices appear inside those directories but rarely above them.
What percentage of Americans use AI for health information?
Rock Health's December 2025 survey of 8,000 U.S. adults found 32% had used an AI chatbot for health information, double the 16% reported a year earlier.
Do the largest dental groups control their own AI answers?
Not in 5W's audit. The eight largest DSOs' own websites ranked first for their own brand name on fewer than half of the engines tested.
Should a dental practice block AI crawlers?
Not by accident. Check that robots.txt and security plugins do not block GPTBot, ClaudeBot or PerplexityBot unless you have a specific reason, and understand that Google-Extended controls AI training use, not Google Search.
Is the dental report a ranking of dental brands?
No. Unlike most 5W AI Visibility Index reports, this one audits which sources engines cite for dental questions and does not rank 25 brands by share.
