AI doesn't remember pharma CEOs the way analysts do. It remembers the category.

Ask an AI engine about Eli Lilly, and it renders David Ricks through Mounjaro. Ask about Moderna, and it renders Stéphane Bancel through the COVID vaccine — still, three years after the commercial cycle ended. Ask about Pfizer, and it renders Albert Bourla through the same vaccine, plus a decade of Medicare drug-price coverage the human news cycle has moved past.

That's the finding of Study 11 in the 5W Reputation Index: Pharma CEOs. Eighteen chief executives. Five AI engines. Seventy-plus retrieval-intent prompts each.

The pharma cohort is the cleanest evidence in the entire Index that AI engines have compressed the CEO role into the therapeutic-category role. Own the category, own the portrait. Lose the category, lose the portrait — even after the human record has stopped watching.

The top of the cohort:

  • David Ricks — Eli Lilly — 82. Series high. Mounjaro. Zepbound. Kisunla. Fifteen years of category-defining approvals inside one Indianapolis institution. Sentiment of 84 and Accuracy of 86 — both the highest single-dimension scores in the pharma cohort. The engines don't render Ricks as the operator of a company. They render him as the personification of a category.
  • Pascal Soriot — AstraZeneca — 74. Fourteen years as CEO. The second-largest oncology franchise globally. The longest-tenured Big Pharma engine portrait we've measured.
  • Emma Walmsley — GSK — 68. The Shingrix franchise. The Haleon demerger. And the cleanest crisis-recovery case in the cohort — the Zantac litigation resolved in 2024 without lasting portrait drag.

The bottom of the cohort:

  • Stéphane Bancel — Moderna — 34. Series low. A $200B market cap at the 2021 peak. A ~90% share-price decline through 2026. Executive departures across CFO, CMO, R&D chief. Sentiment of 26 is the lowest in the cohort. Control of 22 is the lowest in the cohort. External commentary now writes the Bancel portrait — he doesn't.
  • Mike Doustdar — Novo Nordisk — 42. Ten months into his tenure. Inherited a portrait already damaged by category loss to Lilly. The engines attribute the slippage to the incumbent, not the predecessor.

The structural finding: the GLP-1 category advance now writes eighteen pharma-CEO portraits simultaneously. No other sector we've measured produces a comparable single-category effect. A category leader shapes the AI-rendered portraits of every competitor in the category — at once.

AI has become the industry's institutional memory. Every therapeutic-category advance now becomes part of an executive's long-term reputation, whether they want it to or not. Every category loss too.

Boards should be measuring this. Every quarter.
