The Baby Formula AI Visibility Index 2026

Enfamil leads baby formula AI citations at 20.6% while ByHeart's share collapsed to 0.6% after its 2025 botulism recall, a new 5W AI Visibility Index finds.

Enfamil captures the largest share of AI citations in baby formula recommendations, naming leadership across purchase-intent, safety, and comparison prompts. Similac holds the second-largest share, four years after a 2022 recall, while ByHeart's share collapsed within months of its November 2025 botulism recall. Parent's Choice and Kirkland Signature, manufactured in the same Perrigo facilities as national brands, register a fraction of the citation share those brands command.

  • Enfamil, Similac, and Bobbie form the Anchor Tier of baby formula AI visibility, together capturing 49.8% of citations across the fifteen brands tracked.
  • Recency outweighs severity. ByHeart's November 2025 recall, zero deaths, an outbreak the CDC declared over within three months, produced a steeper citation collapse than Similac's 2022 recall, which involved two deaths under investigation and has had four years to recover.
  • Scale buys recovery time. Similac's 15.8% share four years post-recall sits far above Evenflo's 3.6% share six years post-investigation in 5W's Strollers & Car Seats AI Visibility Index, a smaller brand carrying a comparably aged story.
  • Manufacturing parity does not transfer to AI authority. Parent's Choice and Kirkland Signature, both made by Perrigo to the same FDA nutritional standard as Bobbie, combine for 7.0% of citations, roughly half of what Bobbie alone commands.
  • Import-legality questions around European formula brands remain largely unclaimed territory in AI-generated answers, despite active CBP enforcement and FDA warning letters addressing the same question.

Same Factory, Different Answer

Enfamil leads the category at 20.6%, ahead of Similac at 15.8% and Bobbie at 13.4%. ByHeart sits at 0.6%, down from an estimated 5.8% before its November 2025 recall. The FDA's outbreak investigation documented 48 confirmed and probable infant botulism cases tied to ByHeart formula, traced to Clostridium botulinum in an organic whole milk ingredient from a named supplier, with the outbreak declared over by February 2026.

Similac's trajectory is the more instructive contrast. Abbott's February 2022 recall, tied to Cronobacter sakazakii contamination at its Sturgis, Michigan facility and two infant deaths under investigation, remains part of how AI engines describe the brand. But at 15.8% share four years later, Similac still ranks second in the category, a markedly different outcome from Evenflo's 3.6% share six years after a comparable safety event in 5W's Strollers & Car Seats AI Visibility Index. Scale and a visible remediation response buy a national brand more recovery room than a smaller brand gets for a similarly aged story.

The category's sharpest structural signal has nothing to do with recalls. Parent's Choice and Kirkland Signature, Walmart's and Costco's store-brand formulas, are manufactured by Perrigo to the same FDA nutritional standard (21 CFR 107) as every national brand on the shelf. Bobbie is manufactured by the same company. The two store brands combine for 7.0% of citations, roughly half of what Bobbie commands alone. Shared manufacturing and shared regulatory compliance do not translate into shared AI authority; brand narrative and independent content depth do.

This index tracks which formula brands ChatGPT, Claude, Gemini, Perplexity, and Google AI Overviews recommend across purchase-intent, safety, and comparison queries, and how two real, well-documented recalls at different distances in time surface in those answers.

Enfamil leads across every engine tested, with the widest margin in ChatGPT and Gemini responses. Claude and Google AI Overviews return more conservative recommendation sets across the category, consistent with patterns 5W has observed in other consumer verticals. Store-brand and recently recalled products register too infrequently in top-three recommendations to appear reliably in this matrix; their category-wide citation share is captured in the full brand ranking below.

Three Surprises

Store brands lag their own factory

Parent's Choice and Kirkland Signature share a manufacturer and a regulatory standard with Bobbie, yet combine for barely more than half of Bobbie's citation share alone. Brand narrative and independent content depth, not manufacturing origin, determine AI authority.

Scale changes how long a recall lingers

Similac's 15.8% share four years post-recall sits far above Evenflo's 3.6% share six years post-investigation. A larger, more resourced brand with a visible public remediation response recovers more AI visibility than a smaller brand carrying a similarly aged story.

A months-old recall can outweigh a four-year-old one

ByHeart's 0.6% share sits below Evenflo's 3.6%, despite the ByHeart outbreak causing zero deaths versus two deaths under investigation in the Similac case. Recency weighs at least as heavily as severity in how AI engines characterize brand safety.

What Formula Brands Should Build

Publish dated, product-specific recall and remediation pages that cite the FDA's own investigation pages directly, not only a brand's internal statement. Structure ingredient-sourcing and supplier-testing information as citable, dated content rather than general quality-assurance copy. Store brands manufactured by a third party should make the shared-manufacturer, shared-standard fact explicit and citable, since it is not currently reaching AI answers on its own.

Two Open Lanes

Import legality. U.S. Customs and Border Protection has seized shipments of HiPP and Holle formula as misbranded, and the FDA has issued a warning letter to at least one importer for failing to verify foreign supplier compliance. No brand or regulator currently owns a clear, AI-citable answer distinguishing legal personal-use import from unauthorized resale.

Real-time recall status. No brand or neutral source currently owns an AI-citable, real-time answer to "is my formula part of a current recall," despite the FDA maintaining exactly this data.

Commentary

A parent comparing Parent's Choice to Similac is often comparing two products made in the same factory to the same federal standard. AI doesn't know that unless someone tells it. Right now, almost nobody is.

What Brands Should Do Next

Track AI citation share the same way 5W tracks it across other categories. If a brand shares a manufacturer with a national name, make that fact explicit, dated, and citable. If a brand carries a recall, publish the remediation with the same specificity as the original investigation, not less.

Testing Window. Prompt testing was conducted across standardized buyer-intent queries covering purchase intent, brand comparison, recall and safety questions, and budget and store-brand comparisons. Each engine was queried across ChatGPT, Claude, Gemini, Perplexity, and Google AI Overviews, with results aggregated into a citation-share model consistent with the 5W AI Visibility Index series methodology.

Sources. The ByHeart recall timeline and case counts are drawn directly from the FDA's outbreak investigation page. The Similac recall timeline is drawn from the FDA's 2022 Cronobacter investigation page. The FDA warning letter cited in this report was issued to an importer for Foreign Supplier Verification Program violations, not to Kendamil, HiPP, or their manufacturers directly.

Limitations. AI Visibility figures in this report are directional rather than precise measurements, reflecting relative citation-share patterns observed across the testing window. The scoring methodology is proprietary to 5W AI Communications. The Similac-versus-Evenflo comparison spans two separately conducted studies and should be read as a directional cross-study pattern rather than a controlled comparison.
